Rebate for Washing uniform

Recently discovered I might be able to get a tax rebate for washing my work uniform.
My work cloths all have a company logo on trousers, tshirt, sweatshirt, fleece. After reading the page on here about it is that all I have to do just write a simple letter asking for a rebate.
Should I include my national insurance number or anything else?:

You can claim up to the last four years. Last years amount was the tax on £60 which amounts to around £12 a year.
Your tax code is then adjusted so that you pay £12 a year less than yu normally would.
Ring the HMRC, that's all you need to do. They will issue your employer with a new tax code.make the most of it, we are only here for the weekend.

the rate you can claim depends on what industry you work in .
IF you industry is listed hee then you get the tax back based on the applicable figure
http://www.hmrc.gov.uk/manuals/eimanual/EIM32712.htm
If your industry is not listed there then you get the default rate which is £60 x your tax rate so as above 60 x 20% = £12 per year reduced tax provided your uniform ios as you describe so meets the conditions0 -
Just to confirm, if you get the flat rate you wont be getting a refund cheque for the £12 per year for example?
Me and my partner are going through this claim process and hes only been refunded £12, despite 3 years of washing uniform, No cheque as requested, just some confusing tax calculation saying theyve changed our code

post up the calculation and someone will explain it, it really cannot be that confusing - did it look like this one at post 1809 in the main thread on uniform washing, its all been explained before on there0
