Aviva - Expression of Wishes form

I have an Aviva personal pension, and want to check what my current expression of wishes is, and also to (perhaps) update it.

Their website is difficult to navigate to a "documents" section but I did end up here:

http://www.aviva.co.uk/existing-customers/endowment/

which seems to have a link to the form. However, that link 404s:

http://www.aviva.co.uk/adviser/product-literature/view-document.cgi?f=gh04011c.pdf

As there wasn't an online enquiry form or email address, I sent an online complaint stating the above. This was 2 weeks ago and to date I haven't had a reply or even acknowledgment of the complaint - poor show Aviva!

Anyone on here have a working link to this form?

They would accept a letter, doesn't need to be a form.

Besides that, there are 2 versions at the bottom of this page (one of which looks v similar to the one linked above)

EDIT: You have to click 'Product Literature' then 'Personal Pension'

I found that one but discounted it as it said Aviva healthcare on the 2nd page

Make sure your will is uptodate too, that's still the #1 document to sort out your estate/pension once you've met your maker.
