Coventry Mortgage Application delaying exchange!

Hello
Not sure if anyone can give me any advice - might be more of a general rant!
The buyer of my house is a FTB and applied for a Coventry mortgage about 7 weeks ago - and still no offer. The valuation was done almost immediately, and the last I heard the application was having a final review last Friday. But still nothing. Everything else is complete and we are ready to exchange...apart from mortgage offer.
Any ideas what the hold up could be? We have also applied for a Coventry mortagage and received our offer a month ago!
Going slightly stir crazy now...
