You have to give it a little time to load the form - mine went through fine after a little patient waiting!

I found a way to get the form up on Chrome.

But please be careful, because I don't know why it was blocked. It's probably okay, but you might want to think about it. Or contact them and wait for them to fix it.

Look at the address bar, next to the little bookmark star, there might be a shield. Click on that and tell it to run the blocked script, and that brought the rest of the page up for me.
