SSP ended but still off work sick?

I have been off work sick since 8 October last year. Breast cancer treatment. I have been paid SSP since then, sending in GP sick notes to work.
My recent pay slip just arrived and is very low. I called work, it is because SSP is paid for 28 weeks, which is now up.
BTW I am paid a month later than usual - this pay slip is for April.
Looking on HRMC website today, it seems my employer should have sent me notice of SSP ending at 23 weeks of absence, an SSP1 form, I have not received this, or any other letters apart from monthly pay slips in the post.
I understand I can now apply for ESA, but need documentation from work for this.
Work have no knowlege of this form, saying they have done what is required in paying 28 weeks SSP.
I work for a large local call centre - 450 plus employees - but they say they have no experience of anybody being on long term sick leave before.
Where can I go from here? I am getting nowhere speaking with my manager.

    Contact the HR department directly, it is their legal obligation to send you the SSP1. This link https://www.gov.uk/statutory-sick-pay/eligibility says

    "If you don’t qualify for SSP, your employer must give you form SSP1 within 7 days of your illness.

    You can use this to apply for Employment and Support Allowance (ESA) instead."


    Clearly once your entitlement ends you "don't qualify" and they should give you the SSP1.

    Also on that link you can download an SSP1, so your employer can do this directly.

    Your employer can download the SSP1 form from the HMRC website - it is really straight forward to fill in. Failing that, you may want to send them the form with a letter stating that you have been told by HMRC that you need the form to be completed before they can process any ESA claim.

    I would state in the letter/email that you need the form to be completed and returned within 10 working days (this is loads of time as the form takes 10-15 minutes at the most).
