If you click on the menu (the three lines in the corner up there >) and go to "about google chrome" it takes you to a page with a button to report an issue.
It's just a form, you don't need any technical details, just tell them what was wrong.

I reported the same bug to Google a bit ago, seems to have messed up some companies databases which get confused if the radio buttons are unticked and they don't prompt a warning to select at least one option:

Same here on Google Chrome. Glad I saw this thread as I came to ask the question too.
It's also been very erratic on form filling in general and putting things in all the wrong boxes.0 -
Same here! it started a week ago for me. Unticks boxes and its filling in my details incorrectly. Its been driving me mad. And I thought it was just me!0

at first I thought the information needed changing in the autofil settings thing but they all looked fine

I upgraded Google Chrome to the beta version (to deal with another bug involving Pulseaudio on Linux causing audio content to be at a high pitch), version 28.0.1500.45 beta. That version has fixed the unticking of radio and option buttons. Let's hope that fix finds its way to stable versions of the browser soon enough, although haven't had any stability problems with the beta version.0
