After speaking to an incredibly unhelpful person on the phone it would appear this arises from 2010 when I had a number of short term jobs and it appears my employers have all used the same tax code meaning my I overused my personal allowance?

My question is that as I filled in all the necessary paperwork when starting a new job each time it would appear that this is the fault of my employers and not me.

The problem with short term jobs is that HMRC don't have enough time to correct any tax code errors in year.
If you gave a P46 to a new employer and they used the wrong code then by the time they sent it to HMRC and then HMRC processed it you could have moved on to a new employment and so you would not have paid enough tax on that job.
You can appeal the underpayment under Employer Error if you think they did something wrong. You would need to be sure of what box you ticked on the P46's you completed for each new employer.

As regards ESC-A19 you can appeal using this if you think HMRC failed to use information it received from either you or your employer in a timely manner and so failed to stop the underpayment accruing.
