If NI is the same as England & Wales then they are:
Debtors Petition: http://www.insolvency.gov.uk/pdfs/forms/form6-27.pdf
Debtors Statement of Affairs: http://www.insolvency.gov.uk/pdfs/forms/6-28.pdf
Guidance notes:http://www.insolvency.gov.uk/pdfs/forms/debtorsguidance.pdf
Also see: 'How to petition for your own bankruptcy' at http://www.insolvency.gov.uk/guidanceleaflets/dealingwithdebt/howtopetition.htm
Anyone please correct me if NI is different.
EDIT: And Red's link if you want to fill them in online rather than download.

I am currently filling mine out online , very easy and you can see your progress. You still have to print out and file at court , but it all somehow seems more under my control by doing it online ????? may sound strange , hope you get what I mean .
Good Luck Levista ,with it all , and from my short time on these forums , there is always someone can point you in the right direction or give much needed support.BR 08/09/2007 /DISCHARGED 11/04/2008
