Need Help Getting Default Date Moved

Hi,
Quick version:
Been defaulted after being in DMP since July 2009. Want date moved. Need help with letter and address (see towards end).
Got noddle report today and see that I have a new default for 30/3/2013 on "Hfc Bank - HSBC Finance" which is for a HSBC Online Card.
Been on DMP since Mid 2009 with them. (self managed at first them through stepchange since Dec 2010).
Recently they were returned a payment to stepchange saying the account does not exist. I told SC to resend it. Currently they seem to be taking them. This possibly is the reason for default.
The last letter I have off them saying they may default me was Feb 2010. I have had no contact since. In fact when I made my DMP payment myself in DEC due to Xmas pay timings it took over an hour on the phone. Most of it trying to get someone to admit the account existed.
Now default me they can and I have no issue there. What I do have issue with is the date. I would like it moved to reflect the true situation. I have 2 other defaults, one from right at the start of my troubles in July 2009 and one a year later.
QUESTIONS:
1. Is there a template letter around here for moving a default or what should I put in the letter to them.
2. Can anyone advice me where to send it. Since I have had no contact with them in years and they seem to no longer offer the card I am unsure where or how to send it.

Tell them that the ICO's guidance on issuing defaults says they should be issued six months after non-payment and ask them to remove the default from your report completely. They can't re-date it as they need to send a default notice before filing a default, and clearly can't do that if they're trying to lodge it on a past date.

I have sent off a letter a while back and have just today found they have left me a message.
I will update when I am aware what is happening!

And having just recieved a call from them I can confirm the following.
They have agreed to remove the default since the issue was that they returned a payment in, not that I did not keep to the agreement.
All seems good them. However I shall await the confirmation letter and check my credit references in a couple of month to confirm.
