Completing a CRB check form

I wonder if anyone here will know the answer to this. I'm filling in a CRB check form (or Disclosure & Barring service form as they're known now) for a job I started today.

It asks for all addresses I've lived at for the past 5 years. Amongst those will be my 3 different uni addresses. Obviously I've put my home address, where I've lived for 22 years.

The thing that confuses me is the dates, the form says they can overlap but there cannot be gaps. I always came back home for summer so only lived at these addresses for about 8 months at a time.

Since I've put that I've lived at home for 22 years, shall I then just put the uni addresses in and the dates I moved in/out of and leave it at that? Or should I put my home address again between each uni address, if you get what I mean.

    I would put it as home, uni, home, uni etc.... Then there will be no gaps whatsoever.

    That's what I thought and why I asked. When I did one for another employer they got confused when I just put uni dates in and they didn't match up but that was an in-house one and I wondered if this would be different because it's an official form I'm using.

    I moved home from uni, then in with my ex, then home again, then back out again!

    I always that as four separate addresses, even though two of them are exactly the same just different dates. I think that's the clearest way.

    At work, I believe our students put the 22 years at home on just once, with the months in between shown separately.

    You aren't the first graduate to get a CRB, don't stress. Parent's house is normally the main residences for most students, as it is just easier.

    But home address for 22 years and but temporary student addresses.
