Discipline for small accident?

As an aside - although slightly on topic regarding Stanley knives in the workplace.

Where I used to work, someone left an open bladed Stanley knife on top of a pallet of finished goods.

When the fork truck driver placed the pallet into a high rack, the racking frame pushed the knife off the top of the pallet as the driver drove into the deep rack.

Fortunately, the fork truck had protective grills above the cab and the driver escaped injury.

If a Stanley knife has to be used, you can get the type where the blade retracts when contact is lost with the item that is being cut.
