I know this forum deals with bankruptcy so hoping that someone knows an answer to this one, even though it's from the other side.
I've been asked to submit a 'statement of claim' in regard to an employer who has gone into voluntary liquidation.
Does anyone know if this has to be a specific form, or will a detailed letter be OK? I've found a form on the Insolvency website, from a link on this forum 'Form 4.25 Proof of Debt – General Form', do you think this would this be classed as the same thing?

Technically you do not need to use the form. A letter setting out your claim would do.
However, the form would help make sure you include all the info they may need.Free/impartial debt advice: National Debtline | StepChange Debt Charity | Find your local CAB

Think it probably be best to use the 'proof of debt' form then, and just put a covering letter with it.
Will it be OK to email them? Best way to make sure they get there in time and not lost in the post.

OK Thank you for your help.
I've emailed the form along with a letter detailing all the outstanding contracts, I've also scanned a copy of the proxy form so they can't say they haven't received them in plenty of time
