Ever been given a 'report form' after interview?

I attended a job interview as a half day at an assessment centre yesterday and it was hard going as I'd never done anything like it before. I did my best but obviously not sure if i did well or not. It involved role play exercises, written exercises and a competency based interview and you don't know what the right or wrong way of doing any of it is so will just have to wait to see how I did. They said I would hear within the next 2 weeks.
My dilemma is what to do with the 'report form' that they handed me as I left the centre. Basically it's a form asking for my feedback on how I think I did in the exercises and it's in a prepaid envelope. is it part of the interview process?
Do I say i feel i did well, or will this look like I'm too cocky or do i say i could've done better or does this look like i wasn't confident in my performance? You can see my problem. I've never had anything like this given to me before so not sure what their looking for?
I tried to find advice on the internet but this doesn't seem to be a common thing as didn't find much about it other than a small piece regarding de-briefing after an interview and how a report form can be one way of doing it. So could it be that my answers won't matter and its just for the company records to show they ticked the box for debriefing interviewees. What do you think?

I know when I did a graduate assessment thing years ago that the forms didn't form part of the interview process but were simply used to adjust the recruitment process.
That said, I wouldnt want to bet my life savings on the fact that some companies may use it as part of it.
Personally I'd respond, be fairly generic and say that overall you enjoyed it, that it was a good opportunity to meet them and a gave better understanding of the role/ company or whatever as appropriate0
