New Starter Form, who do I send it to?

Hi all!
I've just been sent all the documentation for my new job, contract, terms etc.
There is also the HMRC New Starter Checklist.
Does this form go into the pack of acceptance to my employer or do I have to send it to HMRC?
The employer provided me with a list of things to return and sign.
The New starter checklist is not one of them but I'm not 100% sure where to send it.
