Claim Form has arrived from Court. Now what do we do?

The back-story to this mess is on another thread I started a couple weeks ago but can't find now.

Current situation. Being chased for £ 10,000 credit card debt. At start of this month I sent "Prove It" letter to debt collection agency. They replied along the lines of "Account relates to credit card, we bought debt, here's a copy agreement", which I'll come back to shortly. They also said they were continuing with court proceedings to obtain a Charging Order, but only for security and they wouldn't be enforcing sale of property.

Next day the Claim Form arrives from the Court.

So, what happens next? Does anyone have any advice for us? Is the practically illegible photocopy of the original application form enough to make the debt enforceable, or do they need to produce something more? Is asking for something more likely to upset them, make them less likely to accept small money payments? Really, I'm floundering here. I can't sleep, I barely eat, I've started on medication, and Beachy Head is looking more attractive every day. Any help and advice is gratefully received. Thanks in advance, and thanks again for all your previous assistance.

    Is it the application form you signed? What date did you sign? If it was pre April. 2007 does it have all the prescribed terms?

    First of all confirm you will be submitting a defence and request the hearing is held close to you.

    The application form looks as though it could be the right one, but the quality of the photocopy is so low that most of it is illegible. I can read the date by the signature, which is well before April 2007.

    Sorry to ask another question, but what are "all the prescribed terms"? Could you possibly give me a link? Then again, seeing as I can't read most of the form, I've no way of telling which terms it has or hasn't got.
