Sick pay - 12 mth rolling period-help pls.

Hi I hope someone can help pls.
My employer pays full salary when im off sick for 26wks in a 12mth rolling period. I was off sick for 5wks in July /Aug 2012 then I was diagnosed with breast cancer in Nov 2012. I hav been off sick since then-21wks so far. Obv the two bouts of illness total 26wks and as of May i will get 60pc of my salary. This is paid to my emp'er via a long term disability policy .I am likely to be offfor another few months anyway as i am about to undergo surgery. My questions are what happens when I pass Jul/Aug this year? Do these days get added back to my sick allowance? Also re my annual leave - what happens to it? If i am off sickfor the leave year do I lose it? Normally we are not allowed to c/fwd ann lve.Sorry for the questions but i am v confused atm !
