Change of name on deeds and mortgage

Hello,
I wonder if any of you can help me. Me and my partner have lived in "our" house for 26 months now, we are just about to remortgage. When we bought the house I had a poor credit rating, so it was my partner and my father who took out the mortgage. We have paid the monthly installments and met all our bills, my rating has improved. Now we are in the situation of having to take my father out of the equation as he has split with my mum, so needs to take out another mortgage. When we sorted this when we got the house we were advised this would be quite simple I am just looking for some advise on what to do. Many Thanks
