Tax rebate

Ok, so my employment has been a bit screwed up this tax year.

I was made redundant in May 2012 and so got paid out from that.

I then had part time (ish) employment up to October 2012.

I have been in the same job ever since.

I have just received my P60 for this tax year and have used the calculator on HMRC and it said I could be entitled to a rebate of roughly £680.

Just wondering what I do now? And what the process is?

Nationwide Credit Card ~ £840
Nationwide Personal Loan ~ £1900
