Your work might have a specific form they use as well as this though.
And its 7 calendar days so you don't need a proper note from a doctor.
From what I can see they should have deducted 4 days and paid you 1 day SSP. So they owe you 2 days back and the SSP day.0 -

I'm debating with myself whether the employer should have given the 'why we're not paying SSP' form, but then without the SC2 they may have argued that it was an unauthorised absence rather than a sickness absence.
