How to get Clubcard points from old account?

Hi. Firstly I hope I posted this in the correct place.
I have a Clubcard account which I've not used in a couple years but I know has points on it (about 1000 points). Whatever I try, I cannot gain access to the account online. Put simply, can I take the old clubcard which I still have in to a Tesco store and have the points transferred to my current clubcard?
I've contacted Tesco but they have been less than helpful so hoping someone here will know if it's possible.
