Linked address issue - mortgage rejection

I recently applied for a mortgage with Abbey and was turned down because of "linked addresses" on my credit report. When I investigated I found one was from 5 years ago when my daughter moved out and changed the address on her mobile phone contract which was in my name - she then made the 3 final payments late and it went to a debt collector. It was only for £120 and was settled completely. Now I know about this I can be upfront and explain this to future underwriters.

The other address is more tricky - I lived there following my divorce 13 years ago and ended up with all our joint debt (in my sole name!) and lots ended up defaulting etc as my income was very low and I was getting no maintenance for my 3 children. Eventually the debts put together with a debt management scheme and paid. The linked address was added by Aktiv Kapital and I'm worried that one of the debts may have been overlooked at the time. It was only added 7 years ago but shouldn't all these entries be removed because of their age?

I am very careful with money now and other than this have a clean, carefully repaired credit report. What should I do about advising future advisors about this address link and how is it likely to affect a mortgage application? I should add that I have had 2 successful mortgage applications in the last 3 years with these addresses on my file, I am looking at a second property mortgage which is perhaps why they are being more careful.
Thanks
