Help writing resignation letter.

I need some help with my resignation letter. Basically i have been off with depression since the end of February, this was work related but is a long story so won't go into that. I have now been offered another job so I need to write a letter to my employer to give my notice. I have no idea where to start as i won't be going back to work my notice, I have another 2 weeks signed off. Any help would be appreciated.

Sorry to hear about your depression. Below is my 2 pence worth ...others may think of changing it but in reality if you are resigning and do not require a reference then any simple letter should do. Good luck.
Dear (Name of Manager of whoever is in charge)
I am writing to inform you that after my extended period of illness, I have decided to resign as enable (company/section name) find a permanent replacement to fulfil the role of (insert job title).
My doctor has signed me off sick for the next two weeks, ( please see sick note attached) and therefore I would like to leave your employment on (date to correspond with end of sick note). Therefore I would not be able to come to work to complete any necessary formalities, but I am happy to do so by post (or via a home visit?)
It is unfortunately that I am having to leave in these circumstances, and appreciate your understanding/ support during what has been a very difficult time.
