Need a little help completing a P50 tax rebate form

I need to complete a P50 form to claim back tax from my previous employers on the 01/02/13 (i.e. four weeks after I left my last job), as I'm at university and won't be returning to work until the new tax year at the earliest.
However, I'm mildly confused by the section that asks for Employer PAYE Reference. There's only one box and it doesn't expressly state that it has to be my most recent employer. I left my old job at Computer Exchange in September went I went to uni and went back to work for them over Christmas and should get a P45 from them at the end of this month. However, I never received a P45 from them when I left in September and I don't actually believe I was taken off their records, but I did work part time for Blockbuster during my first term at uni, during which time I believe I was taxed at 20% (I wasn't on an emergency tax code, so I'm assuming HMRC believed it to be a second job). Anyway, should I just use the code for the company I worked for last or should I shoehorn them both in so they've got all the relevant information?

I suggest you get together all your payslips for the tax year and ring HMRC.0
