Don't know about the chart, but if you wouldn't be able to find one, I think (not sure, sorry) in Photoshop there is a way to print picture with the grid on. What you actually do - load the picture into Photoshop, then change in into *.GIF format with limited amount of colours (number you want to use) then print it with grid on.

Another way would be to use a picture like this one, http://www.disney.co.uk/cms_res/disney-junior/images/print_and_colour/pdf/jnp_8.pdf and print it out. Then place and pin it under a piece of white/cream cotton fabric. Using a pen like this trace the picture onto the fabric. http://www.cottonpatch.co.uk/acatalog/info%2d382%2ehtml. Remove the picture and I then pin underneath a thin layer of wadding, or even fleece fabric. Then using the embroidery threads to match the colours you want back stitch all the way round through both layers of fabric. When complete rinse under a cold tap to remove all of the ink. Allow to dry. Then using chalks colour in the picture. The end result will look similar to this, except yours will be Jake. Lol http://www.cedarberrystitches.com/kassie.html

This is called stitchery if you haven't already heard of it.
