I think your company has put themselves in the wrong in the first place by saying you couldn't go back to your old job after maternity leave. You have the right to return to your old job after Ordinary Maternity Leave.

A disciplinary is actually supposed to be an opportunity to help the employee improve to the standard expected. What your employers seem to be doing is telling you that your work is being complained about, not giving you any specifics, and threatening you with a 'probation period'. You couldn't make it up!
