Tax code dropped dramatically due to lease car

Hi,
Can anyone advise me on this matter please?
I have had a lease car since 2006 for working in the NHS in community. I pay directly from my salary a total of £350 this inçludes vat and a handling fee
In May 2012 my tax code has changed to 542L at the time it initially changed to 192k as they said I was new in post and I'd actually been in this post a year!
I have been advised to speak to HMRC who sent me to salary and wages, they sent me to lease car company and all the way back round again! No one will take responsibility and all correspondence state I have a company car. I have never paid company car tax since having a lease car as I pay all personal mileage and a monthly fee of £350 as mentioned.
I have now received a new p11d that states my car is not available to me for 89 days a year and that the cash equivalent of the car would be £2131 based on the cars purchase value of £21675
Please can anyone tell me how to correct my tax code as I am now paying over £5000 per year for a car that isn't mine.
My private mileage is high due to the commute to work hence the cost of the lease. I get 14p per mile for work related visits.
