Filing first Tax Return for Freelance work, do i need to include my FT job details?

Filing first Tax Return (for year 2011-12, leaving it late i know!)
Im employed full time as my main job (paid via PAYE approx £29k per annum), and I have begun doing small freelance jobs in my spare time to earn a couple thousand.
I was originally under the impression that I didnt need to include any of my FT job details in my tax return as I dont receive any shares, pensions from it, and all my income tax is sorted via PAYE.
Only thing I have ever received if a bonus once or twice. (less than £2k)
I am on 'Tailoring my Return' page on the online tax return website and its asking me the following:
In the tax year 6 April 2011 to 5 April 2012:
If you were an employee, director, office holder or agency worker, do you need to complete the employment section?*
Were you self-employed?*
Were you in a partnership?* NO
Did you receive income from UK property including income from land?* NO
If you received any foreign income, do you need to complete the foreign section? Please check help:* NO
If you disposed of any chargeable assets, or had any chargeable gains, or you wish to claim an allowable loss, or make any other claim or election, do you need to complete the Capital Gains section? Please check help:* NO
What do I do for the options in red?
Do i fill out both, or just the self employed bit?

You need to complete both because they will take into account the money earned and tax paid in your PAYE job when they are calculating your final tax bill.0

Can someone help me with self-assessment as well? I don't know what to answer to this:

I am registered as self-employed and the only work I do is for one of these mystery shopping companies. Is that considered being an agency worker? What should I answer to the question above?
