Just got his activation code today ( for the gateway ) but still confused, can't see a form anywhere
He has to tailor the form to suit his circumstances in the 2011/2012 year. There are a series of yes/no questions for him to answer and when he finishes he will be left with the page he actually needs to complete.
If he isn't comfortable doing it himself contact the taxes helpline 0845 3000 627 and see if there is an Enquiry centre nearby which has the facilities to help him do it with one of the advisors there. He can make an appointment to do it.
However I would also advise asking if he actually needs to complete it at all since you say he was in receipt of benefits.
