Help! Problems with my gym membership cancelation!

Hi guys.

I canceled my gym in person and filled out the form and gave it to reception. They said there was nothing else I had to do and my membership would run out on November 1st.

I gave a months notice and the final month was covered by my deposit I paid for when I opened. Confirmed this because I didn't want any fees or nasty surprised.

Unfortunately they didn't cancel! I have been charged for October, Nov, Dec and Jan!

My bank has claimed it back due to direct debit indemnity as I forgot to cancel it (October shouldn't have gone out due to the fact that last month was paid for by deposit)

I am livid. Have they broken any laws? My membership is still active. The receptionist must have lost the form. Any advice would be helpful. I realise I should have cancelled the direct debit earlier but the gym changed companies and the direct debit changed from 'sports and lesuire' to a very strange name on my bank statement and I'm afraid I didn't realise.

Did you not wonder what this 'strange name' which was taking money out of your account.

Didn't get a copy. Just filled out the form and gave it in. There was no copy for me that the gym gives out.

I didn't notice as its my only account and I don't use a credit card so I never notice as so many payments go in and out (My fault I know) But when January showed as coming out yesterday and I didn't know what it was I investigated.

Also I didn't think there would be any issues if i canceled in person. It's not like I sent a letter and it got lost in the post. Lesson learned but I was wondering if there was anything I could do.
