Debt Consolidation loans

Hello,
I owe around 13k through credit cards (2 loans with Halifax) I havnt got a good history, altho the past 6+ months Ive been paying evry bill on time ect.
I just want one loan to clear them all off.
Most big companys wont give me a loan so am wondering if there are any good small companys that can help. I can pay 200-300 per month back.
It will just make things that lil bit easyer for me inthe long run.
