ESA Form Question

Hello, I'm having some problems with my ESA form which, as the computers on the phoneline don't work, I have to complete to make a claim.

Firstly, i don't know how to answer the question 'when did your illness start'. It's been around for a long time, but if i put down a long past date they might question why i'm only claiming now. But it doesn't make sense to write the same date as the date I got the sicknote.

Secondly, the question that asks if you've been working says that if you answer 'no' (i haven't been workign, i was claiming JSA) you have to send in a p45. What p45?

Thirdly, it asks if i've ever claimed Carers Allowance. I have, but that ended in 2005. It seems to be asking if you've claimed it recently, but I don't want to end up giving a false answer.

Anyone know what I should do?

    A few weeks after coming off JSA the DWP Benefits Centre will send you a P45 showing the JSA you have received (just as though you had finished working for an employer).
    A kind word lasts a minute, a skelped erse is sair for a day.

    Don't put off sending it. Just enclose a note stating why you haven't enclosed it.

  • What happens if that slows the process down? I can't wait endlessly for this to be resolved.

    I tried rining the benefit people up but noone answers even though they are meant to be open today.

    I'm at my wits end with this. I've already had to delay the claim by a week because a computer glitch prevents me from making a claim over the phone. I can't wait foreever without an income!

    It won't. I called them up and asked because my dd was making a fresh claim for ESA. Due to a mess up and changing condition. She hadn't received the new P45 through as yet. They accepted as they still had her P45 albeit at a different department. They told me to put a note in saying why I hadn't enclosed the P45...
