Santander self employed form help

I have just received a form from Santander asking for 3 years turnover/ gross & net profit before tax.
I have 2 1/2 years accounts can I use the first 2 years then this years to date (as this years will show easily enough income now the business is established) or am I better doing just 2 and adding a cover letter explaining the dates and current turnover to date?
Also it asks for accountants reference details but I am self assessed?

well no Ive just got the form via email and can obviously ask tomorrow but wanted to get the figures sorted tonight0

They will want SA302's from HMRC if you have no accountant. Do you have them? If not ring and ask.
