Reclaiming charges?!?

Here is my predicament,

My son started private day nursery 4 months ago and we pay £140pcm as a standing order. Four weeks ago we decided to take him out and informed the nursery in writing and they gave us a date for his final visit and the last invoice of £70 as he was finishing half way through the month. All went ok till we checked our bank and found they have taken a full months fee's (£140) and had put the bank account we used for his fee's, into overdraft. The nursery admitted it was the accountants fault and reinbursed us the £70 after 3 days but we are left with £30 worth of fee's for the unauthorized overdraft.

How do we go about claiming this money back off the nursery? or do we claim it off the bank? is the a .pdf form that i can use as a template to send to either nursery of bank? :(
