Help with CCJ removal and for N244

Hi can anyone help me with removing an Unknown CCJ
I recently found out when applying for a credit card that I had a CCJ against my name from 3 years ago! I never received any of the documentation regarding the fine or judgment as I had moved house. The judgment was from the DVLA for a unpaid fine.
I have paid the fine now and received conformation of payment from the DVLA and want to get the CCJ removed by applying to the courts for it to be discounted. I have read that I need to fill in an N244 form and have an affidavit to say I did not ever receive the original warnings/fine.
Does any one know if N244 is the form I need, as when I went along to the county court they seemed to think I could sort it out there buy paying them £15!
If I do need to fill out the N244 can anyone advise on filling it out, I have no idea what to put in the sections “level of judge” and “parties served” …
The form that I have found is here.

Any help would be greatly appreciated
Thanks
