Self Assessment Tax return query

I'm not sure if this is the right place to post so forgive me if it's not! I am currently been on maternity leave and have been employed since September 2010 by the same employer. However last year from July until Feb this year I registered as self employed and carried out 3 private cleaning jobs- hardly worth it I know! Anyway, I am trying to fill in my tax return and have two questions- Do I include the information from my main job on the form? and also it has flagged up on a question that I paid too little tax and therefore my tax code has been changed so do I need to put in the amount I'd underpaid in this section? As my tax code has automatically changed to ensure I make up for this shortfall. Advice/answer much much appreciated! Many Thanks

Yes you include income from your main job. There is a section to fill in for PAYE income. In it you put the amount of tax you've paid through PAYE. HMRC add up the gross PAYE income and the profit/loss from S/E, work out the tax bill and send you it or if below a certain amount, offer the chance to reclaim through your PAYE tax code.0
