help with PIQ form

hi
my OH was made bankrupt last week by HMRC. He is self employed and had not paid his tax bills for approx 4 years.
As he has trouble completing forms I am filling out the PIQB form he has been sent.
We are a family of 5 (2 adults, 3 children). Can you let me know if £200 per month is reasonable for gas and electric charges, as this is how much we spend. Our food, toiletries etc are £600 per month
There is also a question - how much tax do you usually pay each month - how do I complete as he hasn't paid tax for so long? Do I write the amount that his accountant says he should be paying? The same goes for National Insurance.
If I write these amounts down, his outgoings will be more than he earns without extras, such as clothing, haircuts, etc
I appreciate any help. His interview is next Tuesday, 9th October
