Holiday entitlement

I've been on maternity since beg of jan and I'm really confused over how to calculate my holiday entitlement when I'm back at work.
I did work 5 days a week(37.5 hours) and on that basis I was entitled to 24 days holiday this year.
I'm back to work last week of sept (holiday year starts from june) but only going back 3 days but 30 hours so equiv of 4 days..so no idea how to calculate what i'm entitled to...
Also is it correct that I am entitled to the bank holidays that have occurred since ive been on maternity?
