How to writs grievance letter

I have had some problems at work so wish to take things further, how do I word a grievence letter

Read the grievance procedure, to let you know who it should be addressed to (the procedure may require you to raise the matter informally before you submit a written complaint).Ex board guide. Signature now changed (if you know, you know).0 -
Grievances should be raised with a line manager in the first instance in order to give the company a chance to sort the problem out with the minimum of fuss. You should also read your employee handbook on the issue of raising a grievance to ensure you've done everything required of you before raising it. Assuming you have and the grievance is not resolved, then you need to write a letter, headed Formal Grievance. Something along the lines of:
Dear xxxxxx
Formal Grievance
Please take this letter as my intention to lodge a formal grievance against xxxxxx. I have spoken informally with xxxx on (date) to raise these issues but they remain unresolved. My complaint is:
On (date) xxxx did xxxxxx. This was witnessed by
Etc, etc (personally, I would bullet point the reasons for the complaint and keep it completely factual. Do not write "and I thought she was being really mean and laughing at me behind my back" as that won't wash.
I would then write another paragraph explaining what you would like to happen (investigation, meeting, whatever) and end it by saying
As per the employee handbook I understand I can expect a response within xxxdays. I look forward to hearing from you.
