ok, had a little shield I had to click up in the corner of the address bar to allow insecure content and the form is now there.

Cheers Gilla, just entered using IE and it worked perfect. Must just be problems with chrome

The same for me. I've been having problems with the Now FB comps in Chrome, but just entered in IE and it was fine! So yes, it looks like it could be a Chrome problem.
