I've filled everything in and can't figure out what's going on. Anybody else having similar problems?

So what did you put in the field?

it doesnt like symbols I put an & in and got security risk but when i removed it the form submitted ok

I just filled out the ones with the asterisks which are quite a distance away from the form.No need to enter phone number.
