Well, I have done it now, although I am sceptical when they are going to add the points (still waiting for the 100 points from the online survey in January).
And yes, this survey is for everyone as it asks you at the end:
"Please enter the final 11 digits of your nectar card number in the box below so we can credit your account with the bonus points."
It might be worth noting down the ID number you get at the end if you want to complain to them for not adding the points quick enough.Reclaimed thanks to this site:

I have a nectar account with 9 additional card holders. None of us got this email about the survey so I'm bit suspicious as well. Besides, questionpro.com is not a Nectar partner. Why would nectar be conducting a survey for them?
