Have you submitted this TO the IV or was this found during an evidence audit?
If the assessor or candidate doesn't sign it off, what checks and balances and procedures exist to avoid this sort of thing happening?If you haven't got it - please don't flaunt it. TIA.0 -
the work doesn't get printed off it just gets attached to a visit. i am well aware of them shutting us down but i really have not done this. the learner does not sign it and neither does the assessor the feedback is completed on the dicatphone

If no one is signing it off, how does that work in terms of verification? Surely someone has to put their name to it, otherwise things like this would happen all the time?
Once a candidates work is reasdy for submission, all of it needs to be checked, signed off and submitted for verification. Has this not happened?"One day I realised that when you are lying in your grave, it's no good saying, "I was too shy, too frightened."
