How does this letter sound?

Please see the below letter I have not paid my b/card account for 2 moths (token payments of £40 instead of £260) ive had 2 phone calls where they promised to freeze interest , £600 in interest and charges has been added
Can you advise on the letter below? ive tried to keep it factual, but people have said I often come accross as agressive in emails/letters (i'm SO not agressive lol!) any input?

Dear Sirs, I refer to your letter dated XX/XX/XX, which I received on XX/XX/XX. ref account no xxxxx
Unfortunately you seem to have made a number of errors in dealing with my account which I give you the opportunity to rectify.
Error 1 made by yourselves – It Thanks me for my Income and Expenditure report.
I have not sent you an income and expenditure document. You have said over the phone you will send this to me twice to fill in but I have not received anything. Can you supply me with proof of postage if you have this?
Error 2 made by yourselves – You have twice agreed that my interest will be frozen. This has not happened, can you advise when interest will be refunded?
Error 3 made by yourselves – you have verbally agreed that you will accept £150 payment per month in the interim period with frozen interest for a period of 12 months and you will send me this in writing to confirm this. I have not received this letter.
Error 4 made by yourselves – I requested that all future correspondence with me be in writing yet in your letter you ask me to call. I am unable to call you as previous conversations I have had with you have not been actioned. In order to ensure that I have a record of what you have agreed I need this in writing.
In summary I need you to do the following
1. Only contact me in writing
2. Refund the interest you agreed to freeze (records of call are on my account, you have my permission to listen to the calls to hear you agreeing to freeze the interest)
3. Write to me confirming interest is frozen and that you will accept reduced payments in the interim.
4. Let me know if you need an Income and Expenditure report (by letter) I do not need you to send me a form to fill in I already have this as long as you are willing to accept my format.

You have asked me to call by 25th July to avoid further action. As per my request to only be contacted in writing I have written to you by this date.

Please respond by 30th July 2012

Am i covering all legal angles I need to?
How is the tone?
Do i need to add in the legislation reference where they should only contact me by letter if thats what I request?
Anything else I should add?

I think it sounds okay, relatively professional. My only point would be for number4 - just enclose the income/expenditure sheet.
