Go to the disciplinary with the knowledge that you'll be leaving anyway.
It will make the disciplinary seem far more comical, frivolous and entertaining.

Taking a note in from the nurse at the surgery is as much use as taking a note in from your mother.

I don't entirely agree.
A standard sick note had the words "For Social Security Purposes Only" printed on it. I'm out of touch these days but I assume the current "fit note" says the same??
By custom this was largely ignored and it was used to provide evidence of (un)fitness for work for all purposes.
Whilst the unusual and informal note from the nurse (practitioner ??) won't do for SSP purposes it does still provide some evidence that the OP was in fact ill and not just skiving.
Also, most GP's won't provide a fit note during the first week and it is not clear from the OP if that was the start of her illness.
If the OP is being paid SSP and if the employer is a small enough business that they effectively get it back via NI then they have some reason to be upset. However for any other purpose they would be well advised to tread carefully as it is still a professional opinion of sickness.0
