Equifax Credit Report - Associates!

Hi there!
Hoping someone can help me please?
I had an Experian credit report some time ago and I noticed I was still financially linked to my ex (we had a mortgage and joint account together).
We have been split up over 6 years and he is on my Equifax credit report as a financial link and our old address is still in there.
How do I remove him as a link? I certainly don't want my credit rating to be affected by whatever he does.

    File a 'notice of disassociation' with whichever of the 3 credit reference agencies still show you as linked.

    The forms can be downloaded from the websites or, at least with experian, filled in online.
