If you're stressed and feel that having a break from work will help, then take some time off sick (or book some holiday if you have any to spare).
However, be aware that being off may only give you respite rather than deal with the issue(s). Take any time off to seriously think about the problems you're experiencing and try to get a plan of action together to deal with them. This could include trying to tackle work related issues on your return, figure out some coping mechanisms, look for another job or see what your doctor suggests when you go to see them.
