credit score - linked with an ex

Hi guys,
Not sure if I'm posting this in the right place.
My ex has just been in touch because he was checking out his credit score and we are showing as linked. We split about 3 years ago. In 2008 we had applied for a joint bank account, although never completed the process of opening it as I did not have enough of the required ID to show me as resident at his address.
Any ideas how to get this link removed?

You need to get in touch with the credit reference agencies and file a notice of disassociation.
