Need a form to reclaim mortgage interest while on jsa.

Hi,
I have been passed from pillar to post to get hold of a MI12 form to claim back mortgage interest while claiming jsa (i have completed the waiting period of 13 weeks). The jobcentre give me the benefits helpline and the benefits helpline send me back to the jobcentre. Noone seems to want to help.

Hi, I googled MI12 form and this came up to print off the form (dont know if it is the correct form, but does say MI12 on it)
I think my friend just got hers from the jobcentre when she needed one.
