Health Questionnaire with job offer

Hello there...

I received a job offer through the post recently, along with my contract and a health questionnaire.

Does anyone have experience of putting health issues down on a questionnaire? Obviously i want to tell the truth but i am scared that my new employer will wish they hadn't employed me :(

My health issues shouldn't interfere with my work as i've spent some time recovering, and i wouldn't have applied for the job if i didn't think i could do it. Is it acceptable to list the conditions and say that they are now under control?

    Just be completely honest and if your conditions don't affect your capability to do your job you will be fine.
    You may have to follow up with the Occupational Health Advisor, but this is normal.
