Glad to hear that you have got signed off again. It sounds like it would have made you alot worse in going back to work. I know from experience that just thinking about work makes you alot worse when you are signed off. My ex-boss wasnt at all sympathetic, put the phone down on me when I rang in sick/with a sick note and shouted at me and expected the impossible when I tried going back part time.

Just try and forget about work (know its easier said than done) and relax. Look for other jobs in the papers and maybe look into doing temping or something where you could be your own boss - I guess this depends what you area of expertise is

I've been off work for 3 weeks due to depression, most of which is work related. I am due to go back next week but it's really worrying me and i'm loosing sleep because of it.

This concerns me.Your employer has a duty of care, contractually, morally and ethically as well as under various regs (management of h&s at work regs -Regs 3&4, workplace, health safety and welfare regs - i could go on and on). Get you trade union heath and safety rep involved, your employer can't sit back and bear no responsibility for what is occurring to you.
Edit:Your employer must provide you with a safe and healthy place of work, and manage your safety and welfare. Your employer must do whatever is "reasonably practicable" to achieve this.
